Best Medicine Season 2 Episode 4 turns a small-town robbery into an unexpectedly personal case for Martin, with stolen medical supplies, Louisa’s fertility medication and a wounded teenager at the centre of the mystery. The episode also pushes Martin’s complicated relationships forward, as Sarah struggles to accept Eden and Gina’s new pizza truck starts an increasingly ridiculous rivalry with the Salty Breeze.
The trouble begins shortly after Martin’s secret about his fear of blood becomes public knowledge. Rather than giving him any peace, the revelation inspires the people of Port Wenn to play a rather questionable practical joke. Martin is summoned to the Salty Breeze after being told Al has been injured, only to discover that the apparent pool of blood is nothing more than ketchup. It is precisely the sort of joke that sounds funny until you are the doctor standing over it.
Eden, at least, understands why Martin is irritated. Sarah is less sympathetic, particularly because she is still annoyed that Martin kept his phobia from her. Before long, however, the town has a genuine problem. Sal’s Pharmacy is robbed, but whoever breaks in is not particularly interested in cash. Instead, the thief takes gauze, morphine, adult male incontinence products and, most urgently for Louisa, her fertility medication.
That last item gives the case a deadline. Louisa needs the medication within 36 hours or her planned egg retrieval will have to be cancelled. Suddenly, what might otherwise have been a curious local burglary becomes something considerably more important.
Martin and Sheriff Mark Mylow initially follow the strangest clue: the stolen adult diapers. Their attention turns to Collins, whose age and circumstances make him look like an obvious candidate. There is just one problem. Collins did not steal them. He arrived after the robbery, bought the products properly and paid for them. Another promising clue collapses, leaving Martin to wonder why someone would steal medical supplies while deliberately leaving money behind.
At the same time, Sarah decides she needs to make an effort with Eden now that it is becoming obvious that the relationship with Martin is serious. She invites Eden for lunch, presumably hoping for a peaceful introduction. That plan lasts about as long as most peaceful family lunches in Port Wenn.
Eden reveals that she eventually wants Martin to return to Boston with her, and Martin appears open to the idea. For Sarah, who would much rather see her nephew establish a life in Port Wenn, this is hardly welcome news. Things become even more uncomfortable when Sarah tells Eden about Martin and Louisa’s previous kiss, effectively dropping a relationship grenade into the middle of lunch.
Meanwhile, the Salty Breeze has a completely different problem. Gina opens a pizza food truck directly outside the inn, and because she has the necessary permissions and is not technically trespassing, Greg and George have very little legal ground on which to complain. They can, however, complain loudly about customers abandoning the inn for pizza, which they proceed to do.
When someone later breaks into the Salty Breeze pantry and steals alcohol and bug spray, Gina immediately becomes an easy suspect. Martin, however, notices that the theft looks remarkably similar to what happened at the pharmacy. If the culprit is injured and the stolen morphine has failed to provide enough relief, alcohol could potentially be another way of trying to manage the pain.
Then someone breaks into the clinic and steals forceps, making Martin increasingly convinced that the robber is not simply stealing random goods. Someone in Port Wenn is hurt, badly enough to require medical equipment, and is apparently determined not to seek professional help.
Gina unexpectedly provides the breakthrough. George has heard her apparently confess to shooting someone and quickly concludes that she must be responsible for something far more serious than stealing customers from the Salty Breeze. Martin, thankfully, takes a more sensible approach. He learns that Gina has stress-triggered Tourette’s syndrome, causing her to repeat phrases she has overheard.
Her supposed confession was therefore not a confession at all. She had simply repeated something she heard. More importantly, the phrase points Martin towards the woods, where Gina remembers hearing people talking.
The search eventually leads Martin, Mark and Louisa to a remote cabin. Inside, they find the person responsible for the robberies: a teenager who has been badly injured after accidentally shooting himself while showing off with a gun. He and his girlfriend had been staying at his parents’ cottage and, terrified of what would happen if his parents found out, decided to treat the injury themselves.
Their medical strategy is, unsurprisingly, less than ideal. Social media advice is no substitute for a doctor, particularly when the patient has a serious wound and the people treating him have little idea what they are doing.
Martin has to confront exactly the thing he has spent much of the episode struggling with. Despite his blood phobia, he gets to work and provides the teenager with the treatment he needs. It is another reminder that his fear does not make him a bad doctor; if anything, the episode makes his willingness to push through it one of the most important parts of his character.
The mystery is eventually resolved, but Louisa still has her own difficult decision to make. With her fertility treatment depending on the stolen medication being recovered in time, she starts questioning whether she should continue with the procedure at all. Martin does not pressure her. Instead, he helps her work through the uncertainty and reassures her that she is making the right choice for herself.
By the end of Best Medicine Season 2 Episode 4, Martin has recovered Louisa’s medication and gives it to her before the treatment window closes. The episode therefore manages to wrap its robbery mystery around something much more personal: Martin is slowly becoming part of Port Wenn, even if the locals' preferred method of welcoming him involves fake blood and terrible jokes.
There is also plenty going on beneath the main mystery. Sarah's jealousy over Eden is becoming harder to hide, Eden is clearly thinking beyond Port Wenn, and Louisa remains emotionally connected to Martin despite the changes in both their lives. The pizza-truck rivalry is mostly comic relief, but it also gives the episode another way to show how quickly small-town disagreements can become absurdly competitive. Nobody appears capable of simply selling pizza or running an inn without turning it into a personal campaign.
My take is that this is one of the more balanced episodes of the season so far. The robbery storyline gives Martin an actual medical mystery to solve, but the episode wisely avoids making the investigation feel like a standard crime procedural. The teenager's situation also works because the reveal connects nearly every stolen item to the same desperate attempt to hide what happened. Martin's blood phobia continues to be useful rather than simply a running joke, particularly when he has to treat the teenager himself.
I also enjoyed the awkward Sarah and Eden dynamic because neither woman is entirely wrong about Martin's future. Sarah wants him to stay, while Eden represents a life outside Port Wenn. Meanwhile, Louisa's storyline gives the episode some emotional weight without completely taking over. Gina's pizza truck, naturally, is exactly the sort of petty local chaos this show seems to enjoy. Overall, it is funny, surprisingly warm and just messy enough to keep the relationships interesting.
