The upcoming Korean drama Doctor on the Edge (닥터 섬보이) is not arriving with the usual big-city hospital setting viewers have seen countless times before. From romance and comedy to emotional healing and a touch of mystery, the series looks set to turn the small island of Pyeondong-do into the centre of a surprisingly chaotic medical universe.
Premiering on 1 June 2026, the 12-episode medical romance stars Lee Jae Wook, Shin Ye Eun, Hong Min Gi, Lee Soo Kyung, and Kim Yoon Woo. Adapted from the popular KakaoPage webtoon The Persevering Doctor, the drama follows people who arrive on the island carrying emotional baggage of their own, only to discover that the villagers may end up healing them just as much as they heal their patients.
At the centre of the relationship chart is Do Ji Ui, played by Lee Jae Wook. On paper, he has everything figured out. He is a highly skilled plastic surgeon who has spent his life following the elite route expected of him. Unfortunately for him, life has other ideas.
Assigned to Pyeondong-do as a public health doctor to fulfil his national service obligation, he finds himself trapped in the exact place he desperately wanted to avoid. The sea, the island and everything surrounding them trigger painful memories.
What begins as a temporary assignment quickly turns into something much more personal as he forms connections with the people around him. His journey is expected to be less about fixing patients and more about fixing himself.
The person most likely to challenge that emotional wall is Yook Ha Ri, played by Shin Ye Eun. Ha Ri is the sunshine character of the story, at least on the surface. She smiles often, cares deeply about her patients and seems perfectly suited to life on the island.
However, beneath that warm personality sits a secret that has not yet been fully revealed. She previously worked at a university hospital but unexpectedly returned to Pyeondong-do. The mystery surrounding that decision appears to be one of the drama's major storylines.
Her relationship with Ji Ui starts through work, gradually develops through mutual understanding, and eventually grows into what looks like the drama's main romance. Both characters carry emotional wounds, making their connection feel less like a typical love story and more like two people slowly helping each other heal.
Adding another layer to the relationship chart is Hyun Chi Yeon, played by Hong Min Gi. Coming from a prestigious family of doctors, Chi Yeon is intelligent, capable and used to succeeding at almost everything. Unfortunately, feelings are not quite as predictable as medical textbooks.
His apparent interest in Ha Ri creates immediate tension, especially once Ji Ui enters the picture. What could have been a standard romantic rivalry seems set to evolve into something more interesting.

Her personality is sharp around the edges, often appearing blunt and sarcastic, yet beneath that tough exterior is someone genuinely caring.
Jeong Seon looks positioned to provide both comic moments and emotional depth. She also becomes one of the key people connecting the younger medical staff together, even when she pretends she would rather be anywhere else.
The final member of the core group is Yong Ju Cheon, played by Kim Yoon Woo. As a traditional medicine practitioner with a cheerful personality and island-wide popularity, Ju Cheon offers some of the drama's brightest moments.
His youthful charm makes him beloved by residents, but he is not simply there for comic relief. His professional mistakes, personal growth and evolving relationship with Jeong Seon appear likely to form one of the series' secondary romantic storylines.
Their dynamic already feels like the classic case of two people insisting they are annoyed by each other while everyone else can see exactly where things are heading.
Beyond the younger cast, veteran actress Kil Hae Yeon plays O Mi Ja, Ha Ri's grandmother. While details remain limited, she appears to serve as an important emotional anchor in Ha Ri's life and could hold key information about the secrets surrounding the nurse's past.
Looking at the full character map, the relationships form a surprisingly balanced web. Ji Ui and Ha Ri sit firmly at the emotional centre. Chi Yeon creates romantic tension while simultaneously developing a friendship with Ji Ui.
Jeong Seon and Ju Cheon provide a parallel storyline filled with humour, awkward moments and gradual character growth. Around them are the villagers of Pyeondong-do, who are expected to act almost like additional main characters rather than simple background figures.
What makes Doctor on the Edge stand out is that it does not seem interested in portraying medicine as glamorous. Instead of huge operations and dramatic hospital politics, the story focuses on everyday healthcare challenges in an isolated community.
The island setting becomes a character in itself, creating unique situations that urban medical dramas rarely explore. One day viewers may get a heartfelt emergency case, and the next day they may watch the characters arguing over island gossip or getting dragged into local chaos.
Online reactions have been notably mixed but largely curious. Many viewers are excited about the pairing of Lee Jae Wook and Shin Ye Eun, with fans already predicting strong chemistry between the two stars.
Others are intrigued by the unusual public health doctor setting, saying it feels refreshing compared with the endless stream of hospital dramas set in major cities.
